For any insurance plans not listed here, I am considered an out-of-network provider. If your plan includes out-of-network benefits, you may still be eligible for partial or full reimbursement. I’m happy to provide a superbill that you can submit to your insurance company for reimbursement consideration.

Please note that coverage and reimbursement amounts vary by insurance plan. I am happy to answer questions about using your out-of-network benefits and provide guidance on submitting claims.

Cancellation Policy

Please provide at least 24 hours’ notice if you need to cancel or reschedule an appointment, except in emergency situations. Cancellations made with less than 24 hours’ notice, as well as no-show appointments, are subject to a $75 fee.
